The question about mask wearing now vs. early in the pandemic -- early in the pandemic we were told not to wear masks. (What masks I had I gave to nurses at our local ER.) Locally our officials went from shaming people who wore masks to shaming people who didn't overnight. You might want to rephrase that, like specifying "since masks were recommended" as opposed to just early in the pandemic.
I noticed the survey asks about children over 5's experiences, and it seems to make the assumption that the child is in some form of school. Mine is 5 and still preschool age, but currently just at home with me. Just seemed like a hole in that particular part of the questioning, but there wasn't a place to explain
